CSHP Advances National Model of Chronic Care

The Center for State Health Policy was awarded $14.3 million from the Centers for Medicare and Medicaid Services to lead efforts to replicate innovative, cost-effective care management programs in four communities. The project focuses on delivering better care at lower cost to high-need Medicaid, Medicare and uninsured persons in low income areas. Partners in this initiative include the nationally recognized Camden Coalition of Healthcare Providers and The Center for Health Care Strategies.The model will use “hot spot” data analysis to identify and redirect over 2,500 high utilizers of preventable hospital services to care that is more appropriate and less costly. Patient chronic care needs will be managed more effectively and at less cost than hospital-based care, saving a projected $67.7 million over three years.
